تدوین و تعین اثربخشی بسته آموزشی خودمدیریتی درد مزمن بر بهبود خودمدیریتی درد، مهارت‌های ذهن‌آگاهی، باورهای فراشناختی و راهبردهای تنظیم شناختی هیجان در افراد مبتلا به دردهای مزمن اسکلتی-عضلانی

چکیده
پژوهش حاضر با هدف ساخت و معرفی بسته‌ی مداخلات آموزشی خودمدیریتی درد مزمن و ارزیابی اثرات آن بر بهبود خودمدیریتی درد، مهارت­‌های ذهن‌آگاهی، باورهای فراشناختی و راهبردهای تنظیم شناختی هیجان در افراد مبتلا به دردهای مزمن اسکلتی-عضلانی طرح­ریزی شد. جامعه­‌ی آماری پژوهش شامل کلیه افراد مبتلا به دردهای مزمن اسکلتی-عضلانی مراجعه‌کننده به بیمارستان‌های شهر تهران در حدفاصل تیرماه 1399 تا دی­‌ماه 1400 بود. از این میان، 22 نفر (دو گروه 11 نفره) از افراد واجد ­شرایط با روش نمونه­‌گیری داوطلبانه انتخاب و طی یک طرح شبه­‌آزمایشی (پیش‌آزمون-پس‌آزمون با گروه آزمایش و کنترل و پیگیری) بررسی شدند. اعضای گروه آزمایش طی دو ماه تحت 8 جلسه­‌ی مداخله­‌ای برخط 60 دقیقه‌ای قرار گرفتند­ و طی سه جلسه­‌ی 80 دقیقه­‌ای (پیش‌­آزمون، پس‌­آزمون و پیگیری) با استفاده از پرسش‌نامه­‌های درجه‌بندی شدت درد مزمن، خودمدیریتی درد، مهارت­‌های پنج‌وجهی ذهن‌آگاهی، باورهای فراشناختی و تنظیم شناختی هیجان به­ صورت برخط مورد ارزیابی قرار گرفتند. داده­‌ها با استفاده از آزمون تجزیه­‌وتحلیل کوواریانس­ چندمتغیره و مقایسه­‌های تک‌متغیره در بستر آن تجزیه‌و‌تحلیل گردیدند. منطبق بر نتایج، اثرات کلی مداخلات خودمدیریتی معنادار بود (05/0<p). همچنین مداخلات تأثیرات معناداری در بهبود توانمندی خودمدیریتی درد، افزایش به‌کارگیری مهارت­‌های ذهن‌آگاهی و بهبود راهبردهای تنظیم شناختی هیجان نشان دادند (05/0>p اما اثرات معناداری بر کاهش باورهای فراشناختی ناکارآمد نداشتند (05/0<p). با عنایت به شواهد حاصل از این مطالعه مؤلفین بر این باورند که استفاده از مداخلات خودمدیریتی درد مزمن برای افراد مبتلا به دردهای مزمن اسکلنی-عضلانی دارای منافع بالینی قابل­‌توجهی است و لذا استفاده از آن را به متصدیان حوزه­‌ی سلامت توصیه می­‌کنند.

This study aimed to develop and introduce training package of chronic pain self-management interventions and evaluate its effects on improving pain self-management (PSM), mindfulness skills (MS), metacognitive beliefs (MB), and cognitive emotion regulation strategies (CERS) in people with chronic musculoskeletal pain (CMP). The statistical population included all patients with CMP who were referred to Tehran hospitals between June 2020 and December 2021. Among these, 22 people (two experimental and control groups) of eligible individuals were selected by voluntary sampling method and examined during a quasi-experimental design. The experimental group underwent 8 online intervention sessions and were evaluated online by GSCS-7, PSMC-18, FFMQ-39, MCQ-30 and CERQ-18 during tree sessions (pre-test, post-test and follow-up). Data were analyzed by MANKOVA. According to the results, the overall effects of the self-management interventions were significant (p < 0.05). Also the interventions had a significant effects on improving PSM, increasing the use of MS, and improving CERS (p<0.05), however, the interventions had no significant effects on reducing dysfunctional MB (p < 0.05). Considering the evidence from this study, the authors believe that the use of chronic pain self-management interventions for people with CMP has significant clinical benefits, so its use is recommended to health care providers.
