بررسی عملکرد حافظه و هیجان‌های منفی در بیماران تحت عمل بای‌پس قلب در دو شرایط با و بدون استفاده از پمپ قلبی‌ریوی

چکیده
در بسیاری از بیماران قلبی‌عروقی برای برطرف نمودن مشکلات ناشی از این بیماری، جراحی بای‌پس عروق کرونر (CABG) نیاز است. در این نوع جراحی از دو نوع تکنیک استفاده می‌شود که یکی از آن‌ها با استفاده از پمپ قلبی‌ریوی و دیگری بدون استفاده از آن است. طبق مطالعات بیماران بعد از جراحی عوارضی را تجربه می‌کنند که شامل اضطراب، افسردگی و همچنین کاهش در عملکرد شناختی است. در این پژوهش با روش توصیفی‌مقایسه‌ای به بررسی وضعیت بیماران دو گروه قبل و بعد از جراحی و همچنین مقایسۀ عملکرد حافظه که یکی از مؤلفه‌های شناختی است، پرداخته شد. تعداد 62 بیمار به روش نمونه‌گیری در دسترس در این پژوهش همکاری کردند و در ابتدا یک روز قبل و سپس بعد از 2 ماه مورد ارزیابی قرار گرفتند. از پرسشنامۀ سنجش افسردگی، اضطراب و استرس (DASS-21) و از آزمون حافظۀ وکسلر (WMS-R) برای سنجش عملکرد حافظۀ آن‌ها استفاده شد. نتایج نشان دادند که بیماران هر دو گروه در عوامل روان‌شناختی خود قبل از جراحی تفاوتی نداشتند؛ اما تفاوت در مقایسۀ قبل و بعد از عمل بود که گروه با استفاده از پمپ قلبی‌ریوی در عملکرد حافظۀ خود کاهش معناداری نشان دادند. با توجه به این نتایج به نظر می‌رسد که ازکارافتادن موقتی قلب و استفاده از دستگاه پمپ قلبی‌ریوی هنگام جراحی، به افت عملکرد شناختی در بیماران منجر می‌شود. بدین‌روی باید مداخلاتی اندیشید تا بتوان به بهبود عملکرد حافظه و وضعیت روان‌شناختی بیماران قلبی تحت عمل جراحی کمک کرد.

چکیده English

In many of heart patients coronary artery bypass surgery is the best choice for treatment. Two types of techniques are used in this type of surgery. One of them is on-pomp surgery and another is off-pomp surgery. Based on some studies, patients experience some complications after surgery, including: depression, anxiety and decreases in cognitive performance. In this study, the patients psychological conditions and memory functioning which is one of cognitive function are described before and after surgery in two groups. Descriptive- comparative method is used. Total of 62 patients participated in this study, convenience Sampling is used. At the first patients one day before and then two months after surgery were evaluated. Depression, anxiety and stress scale (DASS-21) and also, Wechsler Memory Scale (WMS-R) are used. Results showed that patients in both groups had similar preoperative psychological conditions. The groups showed differences in memory function, on-pomp group showed significant decreases in memory functioning after surgery. According to these results, it seems temporary heart failure and use of cardiopulmonary pump device during surgery, leading to a decline in cognitive function. Because of these, should consider some interventions to improve memory function and psychological conditions of cardiac patients that undergoing cardiovascular surgery.
